The Proof of Residency and Consent Form is a required document for verifying that the applicant:

1. Currently resides at or is legally associated with the property being transferred, and
2. Has received consent from the current rightful owner, occupant, or traditional authority for the ownership transfer to proceed.

This form ensures transparency and consent-based ownership transitions, especially in family-based, community, or customary land arrangements.

• Applicant lives on the property but lacks formal ownership documentation
• Transfer involves relatives, heirs, or shared community ownership
• Consent is needed from:
  • Current occupant or recognized owner
  • Traditional authority (e.g., Chief, Headman)
  • Family representative (for inheritance-based claims)

📎 Contents of the SayPro Proof of Residency and Consent Form:

1. Applicant Details
   • Full name, ID number, and contact information
   • Current residential address and duration of residence
   • Property reference number or description (village name, plot number, etc.)
2. Proof of Residency Statement
   • Declaration that the applicant resides on the property being transferred
   • Supporting documents (select one or more):
     • Utility bill (within 3 months)
     • Letter from the Traditional Council
     • Affidavit signed by a community elder or neighbor
3. Consent Section
   • Full name and ID number of the consenting party (e.g., current owner or heir)
   • Relationship to the applicant
   • Statement giving explicit, informed consent for the property transfer
   • Signature and date
4. Verification by Traditional Authority or Officer
   • Stamp/seal of Traditional Council
   • Signature of verifying officer (SayPro HOT Officer, Community Clerk, or Chief)
