SayPro Documents Required from Employees
(For Participation and Compliance with SayPro Programs and Accreditation Standards)

To ensure full participation in SayPro programs and to maintain compliance with internal accreditation, monitoring, and strategic partnership standards, all employees are required to submit the following documentation as part of the SayPro Employee Identification and Verification Process:

- SayPro Employee Identification Number (SEIN) โ Issued upon onboarding.
- Official SayPro ID Card (Digital or Physical) โ Must be kept updated and visible during assignments.
๐ 2. Personal and Contact Information
- Copy of Government-Issued ID (e.g., National ID, Passport)
- Proof of Residence (e.g., utility bill, lease agreement)
- Updated CV or Resume
- Emergency Contact Details
๐ 3. Academic and Professional Qualifications
- Copies of Academic Certificates (Minimum: secondary education; higher preferred depending on role)
- Relevant Professional Certifications (e.g., police training, public administration, accreditation, etc.)
- Verification Letters or Transcripts (if requested by SayPro Compliance Office)

- Signed Offer Letter and Job Description Acknowledgment
- Non-Disclosure Agreement (NDA)
- Code of Conduct Agreement
- Role-Specific Declarations (e.g., Conflict of Interest, Background Check Authorization)
๐ป 5. Digital Access & Compliance Readiness
- Proof of Access to SayPro Digital Platform (account login details must be activated)
- Acknowledgment of SayPro Digital Tools Use Policy
- Cybersecurity & Data Privacy Agreement
๐ 6. Time-Bound or Program-Specific Documents (e.g., SCSPR-81)
- Assignment Letter or Project Brief
- Compliance Checklist or Self-Assessment Form
- Monthly or Final Report Templates
- Training Completion Certificates (SayPro-issued)

- All documents must be submitted in PDF or scanned format via the SayPro Employee Portal.
- Missing or outdated documentation may lead to delays in project participation, suspension of access to SayPro tools, or non-compliance reporting.
- Employees must notify the HR or Compliance Office of any changes to personal or professional status within 7 days.
