Documents Required from SayPro Employees or Participants
Document Title: SayPro Employee ID and Assignment Form
To ensure proper onboarding, tracking, and task allocation, all SayPro employees and program participants involved in property transfer initiatives must submit the following documentation. These documents are mandatory for issuance of the SayPro Employee ID and activation of the individual’s assignment record within SayPro’s operational systems.
1. Personal Identification Documents
- Certified Copy of National ID or Passport
(Must be valid and not expired) - Proof of Address
(Utility bill, lease agreement, or official municipal letter, not older than 3 months)
2. Employment or Participation Agreement
- Signed Contract or Engagement Letter
(Issued by SayPro HR or the Chiefs Strategic Partnerships Office) - Position Description or Assignment Letter
(Outlines responsibilities, time commitment, and reporting line)
3. Completed SayPro Employee ID and Assignment Form
- This form includes:
- Full Name and Contact Information
- Department and Program Assignment (e.g., Chiefs Home Ownership Transfers)
- Role Title (e.g., HOT Strategic Partnerships Officer)
- Supervisor’s Name and Contact Details
- Start and End Date of Assignment
- Acknowledgement of SayPro’s Code of Conduct and Data Confidentiality Policy
4. Tax and Payment Details
- Banking Information Form
(For stipend, salary, or reimbursement processing) - Tax Number or Tax Declaration Form
(Depending on local tax authority requirements)
5. Additional Compliance and Consent Forms
- SayPro Confidentiality Agreement
(To be signed before accessing any participant or community data) - Consent to Use Digital Systems
(Acknowledging the use of SayPro’s online platforms for data submission and reporting) - Health and Safety Declaration (if fieldwork is involved)
(Especially for on-site work in Chief-led communities)
Submission Process:
All required documents must be submitted online via the SayPro Onboarding Portal or emailed to the assigned regional coordinator. Incomplete documentation may delay ID issuance and assignment activation.
