📌 Purpose:

The SayPro Beneficiary Declaration Form is a required document used to formally declare the identity, eligibility, and relationship of the individual(s) benefiting from a home or land transfer under customary or statutory processes.

This declaration ensures transparency, validates inheritance or transfer rights, and prevents future disputes related to family or communal claims.

✅ When This Form Is Required:

• When the property is being transferred through inheritance
• When the original owner is deceased and a rightful heir is identified
• For multi-beneficiary declarations (e.g., in family or co-owned property)
• Where a legal beneficiary must be named for SayPro registration and municipal linkage

1. Beneficiary Details
   • Full Name, ID Number, Contact Information
   • Date of Birth and Relationship to Previous Owner
   • Community Name and Property Reference
2. Property Information
   • Description of the property or land being transferred
   • Location (village, ward, plot number, GPS if available)
   • History of ownership (prior owner’s name and date of ownership)
3. Declaration of Rights
   • Statement confirming the beneficiary’s claim or nomination
   • Acknowledgement that the property is being received in good faith
   • Declaration that the information provided is true and correct
4. Supporting Parties
   • Signature of Traditional Authority (Chief or Council Clerk)
   • Optional: Endorsement by family representative or community elder
   • Attached supporting documents (e.g., death certificate, family resolution)
5. Legal Consent and Signature
   • Consent to be legally recognized as the property beneficiary
   • Signature of the beneficiary and date
   • Witness signature and ID (where applicable)

• Complete and upload via the HOT Online Portal along with the full case file
• Ensure all signatures and stamps are present before submission
• Only original or certified scanned copies will be accepted

⚠️ Important Notes:

• Fraudulent or contested declarations will result in the case being flagged for legal review
• In the case of multiple beneficiaries, each person must submit a signed declaration or a joint declaration with all names listed
