  • Outreach ID: Unique identifier for each outreach activity.
  • Date of Contact: When the outreach took place.
  • Partner Organization: Name of the external organization contacted.
  • Contact Person: Individual representing the partner organization.
  • Position/Role: Contact’s official position or role.
  • Contact Details: Email, phone, or other contact info.
  • Method of Contact: Email, phone, meeting, video conference, etc.
  • Purpose of Outreach: Reason for initiating contact.
  • Response/Outcome: Summary of partner’s feedback or result of outreach.
  • Next Steps: Planned follow-up actions.
  • Responsible Employee: SayPro staff managing the engagement.
  • Notes: Additional remarks or important details.

Usage Tips:

  • Update log immediately after each partner interaction.
  • Share log regularly with the Strategic Partnerships Office for coordination.
  • Use the log to track response times and engagement effectiveness.
