1. Institution Name
- Full legal name of the medical institution or training provider.
- Include any registered trading or operating names, if applicable.
2. Medical Specialty
- Clearly indicate the specific medical field(s) the institution is seeking accreditation for.
- Examples include:
- General Medicine
- Nursing
- Dentistry
- Pharmacy
- Allied Health (e.g., Physiotherapy, Radiology)
- Public Health
- Emergency Medical Services (EMS)
3. Country
- Specify the country in which the institution is registered and operates.
- If the institution operates in multiple countries, provide a primary location for accreditation purposes.
4. Accreditation Status
- Indicate the current accreditation status of the institution with SayPro:

- ๐ก Renewal (Previously accredited)
- ๐ต Extension (Adding new medical specialties or campuses)
- ๐ด Pending Review
- Include any existing accreditation reference numbers (if available).
